Registration Form Terms & Conditions

Uploading of Supporting Documents

Please upload a copy of the NRIC/WP/In-Principle Approval (IPA) letter for each application for verification. You may also upload a single compressed (zip) file for all applicants.

Important Information

For MSOC/CSOC (EXAM ONLY) courses, trainees must have a valid MSOC/CSOC ID CARD/PASS and must not have failed the course before.

Eligibility for 4 years validity for MSOC/CSOC Card (For Singaporeans and PR Only)

	Metalworking Safety Orientation Course (MSOC)	Construction Safety Orientation Course (CSOC)
Type of Industry	Singapore Manufacturing Industry	Singapore Construction Industry
Worked in relevant industry for 6 years or more	4 years validity on relevant safety orientation card
Worked in relevant industry for less than 6 years	2 years validity on relevant safety orientation card

Payment Mode

All cash payments must be made latest by course date during morning registration by the student or a representative.

All cheque payments must be crossed and made payable to "Concord Associates Pte Ltd" latest by course date during morning registration by the student or a representative.

Please write the Company's Name, Course Title/Date and applicants' name(s) behind the cheque.

Cancellation & Withdrawal

NO withdrawal/postponement/cancellation/refund will be given once the above applicant(s) is/are registered for this course.

Disclaimer

Concord Associates Pte Ltd reserves the right to cancel/postpone this course at short notice and at its absolute discretion without assigning any reason for such cancellation/postponements.

Concord Associates Pte Ltd reserves the right to discard all certificate(s) / ID card(s) that has/have not been collected for more than 3 months from the commencement date.

I hereby confirm that the above applicant(s) is/are able to communicate and write the language registered, and those who do not meet the requirements shall not be allowed to join in the class.
